National Health Fund: Application Form, Eligibility and Documents Required: The “Arogya Pradhan Mantri Cancer Patient Fund (HMCPF)” was established in 2009 under the details of the National Health Fund (RAN).
It is a scheme to provide financial assistance to poor cancer patients living below poverty line for their treatment at 27 Regional Cancer Centers (RCCs). Revolving Fund has been created in all the 27 Regional Cancer Centers (RCC) and Rs. 50 lakh will be placed on his removal.
Cancer patient Rs. Financial assistance up to 2,00,000/- (Rs. Two Lakh only) is processed by the concerned institution/hospital out of the revolving fund placed at their disposal.
Individual cases requiring assistance of more than Rs. 2.00 lakh is to be sent to the ministry for processing.

Benefits of National Health Fund:
- Financial assistance of Rs. 2,00,000/- (Rupees two lakh only) is given for treatment.
List of treatment under this scheme –
- All types of radiation treatment including Radio Therapy and Gamma Knife Surgery/GRT/MRT/Brachytherapy.
- Anti-cancer chemotherapy with adjuvant drugs including hormonal therapy.
- Bone Marrow Transplantation – Allogeneic and Autologous
- Diagnostic procedures – including PET scan.
- Surgery for operable malignant tumour.
Eligibility of National Health Fund:
Eligibility for the Health Minister’s Cancer Patients Fund (HMCPF) within the RAN:
- This fund will provide financial assistance to cancer patients living below the poverty line.
- Treatment support is admissible only in 27 Regional Cancer Centers (RCC).
- Central Government/State Government/PSU employees are not eligible for financial assistance from HMCPF.
- Grant from HMCPF will not be used where treatment/facilities for cancer treatment are available free of cost.
Excluded from National Health Fund Scheme
Central Government/State Government/PSU employees are not eligible for financial assistance from Health Minister’s Cancer Patients Fund (HMCPF).
Where the treatment/facilities for cancer treatment are available free of cost, the grant from HMCPF will not be utilised.

Then the patient needs to fill the application form in the prescribed format.
Once it is filled in completely, the patient or applicant needs to get it duly signed/stamped by the treating doctor and the head of the department and counter-signed by the medical superintendent of the government hospital from where the patient is taking treatment.
After completing all the documentation work, the applicant/s should submit the application form along with the required documents to –

Documents required for National Health Fund:
To avail financial assistance under the National Health Fund (RAN), the patient is required to submit:
- Application form in the prescribed proforma duly signed by the treating doctor and counter-aired by the Medical Superintendent of the Government Hospital/Institution.
- The monthly income of the patient/parents is certified by the Block/Mandal Development Officer/Tehsildar/SDM/Special Officer of the Municipal Board/District Officer, who certifies that the beneficiary belongs to a family living below the poverty line in the area of their jurisdiction, indicating the source. Original certificate of income is necessary for this purpose.
- Copy of Ration Card duly attested by Gazetted Officer with official seal
